The reason charting alone at 2am never works, and what does

A companion to the main ADHD article, written for nurses.

You clock out at 7 and the charting is still open on three patients. You tell yourself you will do it later, alone, when it is quiet. It never gets easier alone.

What this actually is

Body doubling is working alongside another person, in person or on a call, not for help or supervision, just presence. The other person’s attention anchors your brain’s activation system. It is not accountability guilt, it is a nervous system that starts tasks better when it is not isolated.

How it shows up

If you run ADHD or AuDHD, you already know solo charting is where your night dies. You can start an IV under pressure with three alarms going off, but you cannot make yourself open the EHR alone in a quiet break room. That is not laziness, that is initiation load without a co-regulating body nearby.

It shows up in patients too. The ND patient sitting frozen in front of discharge paperwork is not being difficult. Handed a stack of forms alone in a room, task initiation collapses. The same patient, with you sitting beside them walking through it line by line, moves through it in minutes.

What actually helps

Find a charting buddy on shift, even a peer doing their own notes at the same table in silence. Parallel work counts, you do not need to talk.

Use a body doubling app or a video call with a coworker on a slow night if your floor allows personal devices during breaks.

For patients, do not hand off a discharge packet and walk away. Sit with them for the first two minutes of the form, then step out. That initiation boost carries them through the rest.

Stop assuming a quiet private room automatically helps an overwhelmed patient focus. For many ND patients, isolation makes task paralysis worse, not better.

One move today

Ask one coworker to sit near you for the last twenty minutes of charting tonight. No talking required, just two bodies working in the same space.

The fix for the pile of undone charting was never more willpower, it was never being alone with it in the first place.
