Starting is a chemical problem, not a character flaw

A companion to the main ADHD article, written for physicians.

You have thirty seconds between patients and a note that never finished from yesterday. You know exactly what needs to happen next. You still cannot make yourself click into it. That gap is not laziness. It has a mechanism.

What this actually is

ADHD momentum problems live in initiation, not motivation. The ADHD brain runs on interest, urgency, novelty, and challenge, not on importance. A task can be critical and still generate zero internal push to start. Dopamine regulates the starting engine, and when it is low, the engine does not turn over no matter how much willpower gets thrown at it.

How it shows up

In you, if you run this way: sharp mid task, stuck before it. The note that should take four minutes takes forty because starting it feels like pushing a boulder uphill. You mask it well. Colleagues see competence, not the war happening before every task switch. Burnout creeps in through exactly this mechanism, unnoticed for years because you keep clearing the bar in front of witnesses.

In your patients: the one who says, I know I should take the medication, I just do not, is not describing noncompliance. Same initiation gap. Same with the diabetic patient who cannot start the meal log, or the parent who cannot start the intake form. Read it as a systems problem before you read it as a motivation problem, and your differential gets faster and kinder in the same move.

What actually helps

For you: stop starting from zero. Chart the note while the patient is still in the room instead of after. Lower the entry cost, open the tab before you have decided to write a word. Borrow momentum from a task already in motion rather than manufacturing motivation from nothing. Body doubling works, even five quiet minutes next to a colleague can break a charting freeze that willpower alone never touches.

For patients: swap why have not you started this for what is the very first physical action, and help them shrink it. Do not prescribe a lifestyle change as one giant task. Break the referral, the log, the follow up into the smallest possible first move and name it out loud in the room.

One move today

Pick the chart note you are most avoiding right now and open only the template, nothing else, before you leave this exam room.

The fix for a stalled engine is never more pressure, it is a smaller on ramp.
