Someone told you to simplify your schedule and it made everything worse. That is not you failing to follow good advice. That is bad advice failing you.
What this actually is
ADHD overwhelm is not caused by volume alone, it is caused by unmanaged decision load and switching cost. Cutting the list does not fix that, because the brain still has to decide what stays, what order, and when, and that decision layer is often the actual overload. Fewer tasks with the same decision chaos still overwhelms.
How it shows up
In you, if you run this way: you already trimmed your commitments and still feel underwater by 10 a.m. It is not the patient count, it is the constant re-deciding, which chart next, which message first, which of six pending orders needs you this second. That churn is invisible on a schedule and it is what actually drains you before lunch.
In your patients: the ND patient who cannot follow a simplified care plan is often not overwhelmed by the number of steps, they are overwhelmed by the ambiguity in them. Take one pill a day sounds simple until they have to decide when, with what, and what happens if they forget, three decisions hiding inside one instruction.
What actually helps
For you: fix order, not just quantity. Batch same type decisions together, all refills at once, all messages at one set time, so you are not switching decision categories every four minutes. Build defaults, a standing rule for common scenarios removes a decision entirely rather than shrinking it.
For patients: replace open instructions with fixed anchors. Not take it in the morning but take it right after you brush your teeth. Remove the when-decision by tying it to something that already happens automatically. Write the plan as a sequence, not a summary.
One move today
Pick one recurring decision in your day, refill requests, and build a standing rule for it before your next shift.
Overwhelm is a decision problem wearing a to do list costume, fix the decisions and the list gets lighter on its own.
