A companion to the main ADHD article, written for nurses.
You get home from a double shift and your partner, also ND, has also had a day. Neither of you can remember whose turn it is to figure out food, and the silence in the kitchen is not peace, it’s two depleted nervous systems circling the same problem.
What this actually is
When both partners in a relationship are neurodivergent, there’s no built-in neurotypical partner absorbing the executive function slack, masking the mismatch, or translating between two different processing styles. Both people run time blindness, both run RSD, both run sensory limits, and the household has to be built around that reality instead of pretending one of you will eventually become the \”normal\” one.
How it shows up
If you’re ND and your partner is too, you already know report-level masking doesn’t stop at the hospital doors. You come home still performing composure, and so does your partner, and two masked people trying to unmask around each other at 9pm after twelve hours of holding it together can misread exhaustion as distance. Time blindness compounds: you’re both late, both surprised, both defensive.
With patients, this shows up in couples where a caregiver partner is also ND and gets read by staff as \”unhelpful\” or \”checked out\” during discharge teaching, when actually they are as overwhelmed by rapid verbal instructions as the patient is. Assuming one support person in the room has spare capacity is often wrong.
What actually helps
At home, replace vague division of labor with explicit, written systems, whoever’s brain is less fried that day owns dinner, not whoever \”should.\” At work, when teaching a couple where both partners seem ND, slow down and write everything down for both of them, don’t assume one is the neurotypical backup. Watch for two people mirroring each other’s dysregulation instead of one regulating the other, and offer yourself as the calmer body in the room when needed. Ask directly who processes information best in what format instead of assuming the partner is the fallback translator.
One move today
Next discharge with two visibly overwhelmed support people, write the three most critical steps on paper for both of them, not just the patient.
Two ND partners can build a great life together, but only with systems, not assumptions.
