A companion to the main ADHD article, written for physicians.
You have a patient in front of you with fatigue, dizziness, joint pain, and a stack of normal labs. Before you write anxiety in the chart, read this.
What this actually is
Ehlers-Danlos syndrome, POTS, and autism plus ADHD (AuDHD) show up together at rates too high to be coincidence. Connective tissue laxity, autonomic dysregulation, and neurodivergent wiring appear to share developmental threads. This is not three unrelated diagnoses politely coexisting. It is one nervous system, described from three different specialties.
How it shows up
If you are ND yourself, you may be the physician who pushes through tachycardia on rounds, blames joint pain on being tired, and never connects your own hypermobility to your own sensory sensitivity. Masking a body that is struggling on top of masking a brain that works differently doubles the load, and nobody on your team sees either one.
In patients, this cluster hides behind vague, shifting complaints: dizziness on standing, pain that migrates, sensory sensitivities dismissed as quirks, GI issues nobody can pin down. These patients get labeled anxious or somatic long before anyone asks about hypermobility or sensory profile.
What actually helps
Add the Beighton score to your exam reflex when a patient presents with unexplained fatigue plus autonomic symptoms. Ask about family history of double jointedness and sensory sensitivity in the same intake, since they cluster.
Check orthostatic vitals or order a tilt table before reaching for a psychiatric explanation. Screen for ADHD and autism traits in patients already diagnosed with EDS or POTS, not only the reverse.
If you suspect this pattern in yourself, get your own orthostatics checked by a colleague. Do not self-diagnose off a symptom list between patients.
One move today
The next time a patient’s labs come back clean but the story does not add up, run orthostatic vitals before you reach for the word anxiety.
A body that is loose in the joints and loud in the nervous system is not being dramatic, it is telling you the truth in a language your training may have skipped.
