What Is the Interoception Gap?

The Interoception Gap is the reliable gap between what the ND body is signaling and what the ND brain receives. Internal data (hunger, thirst, bladder, fatigue, pain, overwhelm, emotion) is either not registering at all or arriving in a sudden, delayed flood. The gap operates across three layers (Detection, Interpretation, Action) and is the underlying mechanism that makes ND executive function tools fail when built on neurotypical assumptions about body awareness.

For neurotypical brains, interoceptive signals climb in graduated volume. Hunger moves from a 2 to a 4 to a 6 and the brain responds at 5. For many neurodivergent brains, that graduated signal does not arrive. The signal is muted, then it arrives at maximum volume with no warning ramp.

You are not bad at self-care. You are missing a signal.

The Three Layers

The Interoception Gap can fail at any of three independent layers. The intervention is different for each.

  • Layer 1. Detection. Can you feel the signal at all? For many ND brains, low-grade hunger, mild thirst, early fatigue, and rising overwhelm simply do not register. The signal is there. The detection is not.
  • Layer 2. Interpretation. When you do feel something, can you tell what it is? ND brains routinely confuse hunger with anxiety, thirst with restlessness, fatigue with boredom, overwhelm with anger. The signal arrives, but it gets mislabeled.
  • Layer 3. Action. Even when you detect and interpret correctly, can you act on it before the signal escalates? Executive dysfunction lives here. You know you should eat. You cannot make eating happen until the hunger is at a nine.

Your recovery plan has to know which layer is failing for you, because the intervention is different at each one.

What This Looks Like in Daily Life

  • You sit at the desk for six hours, then stand up and your leg gives out. You did not feel any of the warning signals.
  • You feel “anxious” all afternoon, eat something, and the anxiety lifts. The Layer 2 mislabel was the whole problem.
  • You know you should pause the task. You also cannot pause the task until pressure is at a 10. Layer 3.
  • You go to the ER for dehydration because your thirst signal never arrived at a 4 out of 10. It went from 0 to 9.
  • You “crash out of nowhere” after a conference. The interoceptive bill was being added silently all weekend.

Why “Listen to Your Body” Fails ND Brains

Most mainstream interoception advice is some version of “do a body scan” or “be more mindful of your physical sensations.” For ND brains, this is roughly as useful as telling someone with low blood sugar to “be more aware of their glucose level.” The advice assumes the signal is already loud enough to scan.

For ND brains, the signal often is not there at Layer 1, gets mislabeled at Layer 2, or cannot be acted on at Layer 3. Building bigger internal awareness will not help. Building reliable external scaffolding will.

What Helps

  • Timer-based body checks, not feeling-based. 90-minute timer with the same four-question check every time: hungry, thirsty, bathroom, tense. You are not checking your feelings. You are checking the data points.
  • Eat and drink on a schedule, not on hunger or thirst. Detection cannot be your trigger.
  • HALT check. Hungry, Angry, Lonely, Tired. A reliable Layer 2 translation key.
  • Cycle tracking, if you menstruate. The same fatigue reads differently in follicular vs. luteal weeks. Tracking the cycle gives you the modifier on the data.
  • Body double the boring action. Eating with another person on a video call, drinking water during a coworking session. Borrowed Activation is the Layer 3 intervention.

The Bottom Line

If your body has ever ambushed you, if you have ever been “fine” right up until you were not, if every recovery plan has assumed you could hear what your body was saying, the framework you needed was the Interoception Gap. The advice was wrong. The signal was the problem. The fix is external scaffolding, not bigger internal awareness.

You Are Not Broken. You Are Not Too Much. You Are Smart and Capable. You Can Do Anything with the Right Tools.
