📅 The First 90 Days After Diagnosis Checklist

A checklist from ND Hive. Companion to “Late-Diagnosed ND. Now What?”

What this is

The first 90 days after a late ND diagnosis (or strong self-recognition) are disorienting on purpose. Your identity is reorganizing. Most people try to “fix” everything immediately and burn out. This checklist gives you a slower, sustainable order.

Takes 10 minutes to read. Use as your reference document for the next 12 weeks.

Days 1-30: Just process

The first month is for letting it land. Resist the urge to restructure your whole life.

Allow

  • ☐ Crying about your younger self
  • ☐ Anger at people who missed it (parents, teachers, doctors, exes)
  • ☐ Grief about decisions you made without this information
  • ☐ Reading obsessively
  • ☐ Finding ND community online
  • ☐ Telling 1-3 trusted people, no more

Don’t yet

  • ☐ Don’t disclose at work
  • ☐ Don’t restructure your career
  • ☐ Don’t end relationships
  • ☐ Don’t make major financial decisions
  • ☐ Don’t “fix” yourself
  • ☐ Don’t pressure yourself to be a “good” ND person

Practical

  • ☐ Bookmark 3 trusted ND content sources (skip the bad ones)
  • ☐ If diagnosed, pick up any prescribed medication and start the conversation with your doctor
  • ☐ Sleep 8 hours where possible
  • ☐ Eat real food, regularly

Days 31-60: Start naming + small experiments

Now you can start translating recognition into specific awareness.

Identity work

  • ☐ Make a list of moments in your life that make sense now
  • ☐ Make a list of moments that you previously blamed yourself for that weren’t your fault
  • ☐ Talk to one ND adult further along than you (community, online, friend)
  • ☐ Read at least one book or long-form piece by an ND adult
  • ☐ Start noticing your specific patterns (when you mask, when you crash, what triggers what)

Systems experiments

  • ☐ Try ONE new external scaffolding tool (calendar, task list, body doubling)
  • ☐ Try ONE sensory adjustment (headphones, light dimming, fidget)
  • ☐ Try ONE social adjustment (canceling something low-stakes, asking for accommodation)
  • ☐ Note what helps + what doesn’t

Relationships

  • ☐ Have ONE conversation with someone close about what’s changing for you
  • ☐ Notice who can hear it without minimizing
  • ☐ Notice who can’t (this is data, not judgment)

Days 61-90: Strategic decisions begin

By Day 60, you’ve absorbed enough to start making informed choices.

Disclosure decisions (use the Disclosure Decision Tree if you haven’t)

  • ☐ Decide whether/who to disclose to at work
  • ☐ Decide whether/who to disclose to in family
  • ☐ Decide what to keep private (this is also valid)

Workplace decisions

  • ☐ Identify ONE accommodation that would help (formal or informal)
  • ☐ Decide if your current role is sustainable in its current form
  • ☐ If not, start exploring (don’t quit yet)

Support decisions

  • ☐ Find/start with one professional support (therapist, coach, doctor, peer support)
  • ☐ Choose one ND-friendly community (Hive, Discord, in-person, etc.)
  • ☐ Identify your “any time” person (the one you can text “I’m stuck” to)

Health decisions

  • ☐ Establish baseline with a doctor who understands ND (or find one)
  • ☐ If on medication, evaluate after the first 60 days
  • ☐ Address any health stuff you’ve been putting off because of executive dysfunction

Things to NOT do in the first 90 days

  • ❌ Don’t tell everyone immediately
  • ❌ Don’t quit your job (unless it’s actively harmful)
  • ❌ Don’t end your relationship (unless it’s actively harmful)
  • ❌ Don’t try every productivity system at once
  • ❌ Don’t measure your healing on a timeline
  • ❌ Don’t compare your experience to other people’s
  • ❌ Don’t “should yourself”, this is recognition, not optimization

At Day 90, ask yourself

What do I know now that I didn’t know on Day 1?

What’s working that I want to keep?

What’s NOT working that I want to change?

Who has earned more access to me?

Who needs less access to me?

What’s the next 90-day question?

Reminder

The first 90 days is groundwork. Not transformation. The transformation happens in the year after, sometimes the years after that.

Your job in the first 90 days is to stay alive, stay safe, build one or two real connections, and not blow up your life out of grief.

The version of you in 6 months will thank you for not making the big moves before you knew yourself.
