A companion to the main ADHD article, written for nurses.
It’s hour eleven, your notes are three patients behind, and you have stared at the same open chart for six minutes without typing a word. You are not lazy, your brain needs another human present to start.
What this actually is
Body doubling is working alongside another person, in person or on a call, to activate task initiation. For ADHD brains, the presence of another person, even silent, lowers the activation energy needed to start something the brain is otherwise stuck on. It is now FSA and HSA eligible with a letter of medical necessity.
How it shows up
In you: you can knock out charting fast when a coworker is nearby doing their own notes, and completely stall when you’re alone in the break room with the same task. That’s not motivation, it’s initiation, a distinct executive function that body doubling directly supports.
In your patients: the patient who can’t start their discharge paperwork, PT exercises, or medication setup alone at home isn’t unmotivated, they may need exactly this, someone present while they begin. Families often mistake this for laziness and push independence too early.
What actually helps
For you: if your workplace offers an FSA or HSA, ask your prescriber for a letter of medical necessity covering body doubling services, then use a virtual body double app or service during your worst task, likely charting, during a slow stretch of shift.
For patients: when discharging a patient who struggles to start tasks alone, mention body doubling as a legitimate, reimbursable tool, not just a tip. Suggest they start their first dose or first exercise with a family member simply present, not helping, just there.
One move today
Look up whether your FSA or HSA plan covers a letter of medical necessity, and ask your prescriber about it this week.
Starting a task alone is a specific brain function, not a willpower test, and paying for a body double is no different than paying for glasses because your eyes need the assist.
