A companion to the main ADHD article, written for physicians.
You’ve prescribed medication, therapy, maybe a coach, and still watched a patient’s executive function collapse the second they’re alone with a task. Body doubling is the intervention nobody trained you to write a letter of medical necessity for. Time to fix that this week.
What this actually is
Body doubling, working alongside another person, in person or virtually, purely to activate task initiation and follow-through, is FSA/HSA eligible when it’s supported by a letter of medical necessity tying it to a diagnosed condition like ADHD. It’s not a lifestyle indulgence or a productivity gimmick. It’s an external regulation tool for a brain that reliably struggles to self-initiate without another presence acting as a low-key accountability anchor, and the research on task initiation in ADHD backs the mechanism up.
How it shows up
If you’re ND yourself, you already know why body doubling works because you’ve probably done it without ever having a clinical name for it: called a colleague just to feel less alone while finishing notes at 11pm, or dragged your laptop to a coffee shop because solitude alone kills your momentum stone dead. Your ADHD patients describe the identical mechanism constantly, a task initiation failure that resolves the instant another presence is in the room or on a call, even when that other person does nothing but exist nearby. It’s a real, mechanism-based intervention rooted in dopamine and accountability circuitry, not a workaround for weak willpower or poor discipline, and patients deserve to hear that framing from you explicitly.
What actually helps
Write letters of medical necessity that name body doubling specifically as a therapeutic strategy for executive dysfunction tied to the underlying diagnosis, so patients can actually use FSA or HSA funds for coaching or dedicated body-doubling services instead of paying entirely out of pocket. Ask patients what actually gets tasks started for them in practice, not just what they intend to do, since intention and initiation run on genuinely different neural systems in ADHD and treating them as one thing wastes everyone’s time. Normalize the tool clinically in your language instead of treating the request as unusual or slightly embarrassing. And if solitary work quietly tanks your own output more than you’d admit out loud, borrow the intervention yourself before you write the next chart note alone at midnight, no diagnosis required to try it.
One move today
Draft one template letter of medical necessity for body doubling services this week so you’re not starting from a blank page the next time a patient asks you for one.
The tool isn’t a crutch someone should outgrow. It’s the missing scaffolding for a brain whose initiation system genuinely needs a witness.
