You have tried four productivity systems and abandoned every one of them by week two. That is not a discipline problem. That is a system mismatch.
What this actually is
Most productivity systems are built for brains that run on stable motivation and linear planning. ADHD and autistic brains run on interest, urgency, novelty, and dopamine, not willpower. A system built for the wrong operating system fails every time, no matter how smart or driven the person running it is.
How it shows up
If you are ND yourself, you know this pattern intimately. Color coded planners abandoned by Thursday. A task manager with four hundred unread items. You can run a code flawlessly under acute pressure and still forget your own expense reports for six months. That gap is not laziness, it is an interest and urgency based attention system doing exactly what it was built to do. In your ND patients, the tell is similar. A patient who has failed five diet plans, three medication schedules, or every chore chart handed to their kid is telling you the system was wrong, not that they lack willpower.
What actually helps
Build around urgency and body doubling instead of fighting them. Use external, visible deadlines rather than internal ones, since ADHD brains respond to now versus not-now, not soon versus later. Pair tasks with a person, a timer, or a live session instead of a silent to-do list. Break tasks into the smallest possible entry point, not the whole arc. For patients, swap “have you tried a planner” for “what is the smallest version of this you could start in two minutes.” That question surfaces the real barrier, usually initiation, not information or motivation.
One move today
Pick one task you have avoided for over a week. Set a 10 minute timer, tell one colleague you are starting, and do only the first physical step, nothing more.
The fix was never more willpower, it was a system that matches how your brain actually fires.
